Suspension Straps

Suspension straps are a great workout tool to help develop proper motor control. There are a variety of models on the market, but the one that we recommended is the Jungle Gym (check out the exercise aides section). They are incredibly convenient and can be set up pretty much anywhere. The ability to leverage gravity and your bodyweight makes it easier to balance, allowing you to focus more on using proper technique. Suspension straps can really help you properly engage the right muscle groups when performing exercises such as lunges and squats.

Initial Setup:

-Place strap anchors over the top of an open door away from the door frame about shoulder width apart




-Secure straps by closing door firmly, pull on straps to check





Squats:

-With legs slightly farther than shoulder width apart, knees pointing slightly outward and holding on to straps, lower buttock towards ground


-Make sure that you lean back, keeping your shins vertical over your feet

-Repeat for 10 repetitions

Notes:
-To ensure that your knees don’t collapse inward, make sure that they are externally rotated (away from your belly button)

-You should notice your buttocks engaging throughout the movement

-Make sure you take advantage of the straps allowing you to range past your weak point

-Once you feel comfortable enough with this exercise, you can make it more advanced by doing it on only one leg. There are a couple variations that you can do, one where you simply lift your leg straight in front of you while you do the squat (single leg squat), or you can cross your leg over your opposite knee (flamingo squat).






Lunges:

-Standing up holding straps in both hands, step directly back with right foot

-Lower your body so that your buttock moves downward and backwards, not forward

-Stand back up and return right foot to original position

-Repeat 10 repetitions

-Switch sides

Notes:

-pay attention to movement of front knee, make sure it moves straight up and down, not deviating side-to-side, the straps should help with this, but if you having trouble, move slower and don’t lunge down as far


-concentrate on sitting back, rather than leaning forward



Betting on patients- the one thing we can count on

Today I saw one of my favorite patients- a lovely 36 year young gal who herniated a disk in her back in a work injury

In reviewing my notes from an initial visit, I had commented that she had:
  • positive factors predicting good outcome: very pleasant, still working, non smoker, nice skin (marker for overall connective tissue quality), smart, well motivated 
She had one major negative factor- she is morbidly obese, with a height of 6' 0", and a weight of 350 lbs

We discussed different treatment options, including physical therapy, injections, and surgery, but I emphasized that the main predictor of a patients outcome is the patient themselves, and not their anatomy.  It would be easy to focus on her weight, but she has so many wonderful positives, we made a point of focusing on those, while giving her specific tactics for weight loss.

I saw her again today, and she was just as wonderful as I had remembered her ..... except that she was 30 lbs lighter in a period of just 4 months!  
- she is working with a dietician
- she is exercising regularly
- she is focusing on portion control

Such a wonderful reminder that when we bet on our patients and give them an opportunity to optimize their health, they can do amazing things.

Meet the Team: Laura Cassidy



How did you become interested in Health, Wellness and Fitness?
My interest originated from my involvement in sport.  My parents started me with softball at a young age, but my true sporting passion was field hockey (I played at the high school and collegiate level).  Athletics taught me the importance of staying fit and sparked my interest in studying human movement and exercise physiology.  I went on to earn a BS in Kinesiology and now work for two wonderful musculoskeletal physicians! 

What are your favorite aspects of working in small, intimate practice?
Flexibility and opportunity. 

I appreciate being able to implement changes and watch them in action.  Whether it’s a tweak to our patient policies, adding a blog to our website or researching and providing training tools to keep our employees happy and motivated, my position at LWSS allows me the freedom to do so.  My position here has also given me the opportunity for personal and professional growth beyond my expectation.

What's your favorite form of exercise?
I couldn’t pick just one!  I enjoy running (I completed my first marathon in October), heated Vinyasa yoga and pilates.  I like to vary my routine and keep challenging myself.

What is your favorite way to integrate a healthy lifestyle into your work day?
Personally, I find that exercising in the morning before work is the best way to start the day.  While at the office, I make sure to drink a lot of water, eat small meals with a good balance of protein/ carbs and stretch multiple times to keep energized.

How would you define the brand at Lake Washington Sports & Spine?
The LWSS brand is all about providing targeted, high-quality, effective musculoskeletal care.  Our clinic takes great pride in the fact that we are a small, physician-owned business.  What that means for our patients is that they’ll never have to deal with a phone tree, they’ll be greeted by the same warm, friendly faces each time they visit our office and most importantly they will have access to physicians who are able to utilize their skills and knowledge without the constraints inherent in a large system. 


You have worked in other physician offices, including other Physical Medicine & Rehabilitation offices, before.  What makes Lake Washington Sports & Spine different?
Our positive culture, for sure. 

What are the deliberate tactics you use to foster that positive culture?
The positive culture at LWSS is directly related to our promotion of personal accountability.  Our culture revolves around being the best person/employee you can be every day, which means taking ownership of mistakes, asking for help when necessary and being a supportive team player.  It’s not necessary to be a manager or a physician to be a leader; LWSS functions so well because each of our employees is a leader in their own role.

You yourself enjoy being active.  What are the things you seek in a physician as an active, athletic professional, and how does Lake Washington Sports & Spine succeed in providing that kind of service?
Choosing a physician is a difficult task and more complex than simply choosing a doctor alone.  Personally, I make my decision based on three factors: the physician, their schedule and their office staff.

 First, I look for a physician with solid experience in his/her specialty, good bedside manner and most importantly someone I feel comfortable with. 

Second, I like to check the doctor’s availability.  Do they have convenient hours?  Is the wait time for an appointment reasonable? I appreciate someone who is flexible.

Last but not least is the physician’s office staff.  It’s so important to me that staff is friendly and competent.  Fantastic staff is the cherry on the sundae!

What advantages are there for patients in having 2 physicians, Dr. Hyman and Dr. Chimes, who are both boarded in Sports Medicine, perform spine injections, perform nerve tests, perform musculoskeletal ultrasound, and experts in exercise?
Well, quite simply, two (brains) are better than one! 

Although Drs. Hyman and Chimes are both physiatrists, they each have areas of expertise, specialization and personal interest.   Having two high-caliber physicians in our office allows patients to benefit from each physician’s depth of knowledge.  The doctors often discuss challenging patients, share interesting ultrasound findings and use each other as a sounding board.

Do you feel empowered to help mold the practice to improve the patient experience?
Absolutely!!

Meet the Team: Tony Trinh!

Let's meet Tony Trinh, Medical Assistant Extraordinaire!




When did you first become interested in musculoskeletal medicine?
-Participating in sports growing up and being a huge sports fan, there is nothing more frustrating then getting injured or seeing your favorite athlete injured. This really spurned my interested in the mechanisms of injury and the process of getting back to the field, and for the non-athletes, just back to every day life.
You are certified in Functional Movement Screens (FMS).  How does that influence the way you think about patients?
-The Functional Movement Screen is a ranking and grading system that looks at movement patterns that are key to normal function. It helps identify functional limitations and asymmetries that can predispose a person to injury and reduce the effects of functional training and physical conditioning. The most important thing that I was able to take away from FMS is that the body, and all of its parts, work together as a functional unit. When thinking about patients, pain may be localized to a specific area or body part, but it is likely that other structures or movement patterns are also involved and contributing to their discomfort. For example, if a patient is having pain in their shoulder or elbow from playing tennis, although it may appear straightforward and localized, an overcompensating movement pattern due to limited hip flexibility could be the underlying issue.
What are your thoughts about role of Musculosketal Ultrasound and how it improves the patient experience in treating musculoskeletal conditions?
-I had not been exposed to the use of ultrasound in a musculoskeletal capacity before coming to Lake Washington Sports and Spine, but seeing it in action, I was immediately impressed. Not only does it allows you to see structures within the body in incredibly high detail (you can see the individual fibers of tendons!), it does this in real time, so you can observe body structures with dynamic movement. Something that is also particularly help is that the patient can give Dr. Chimes and Dr. Hyman feedback in real time, letting them know whether or not the positioning of the probe is painful, allowing them to discern which irregularities are concordant with their symptoms and which ones are simply incidental. 
What is different about the way that Dr. Chimes and Dr. Hyman treat patients compared to your prior exposures to sports medicine? (What are your favorite aspects of how Dr. Chimes and Dr. Hyman educate patients?  Can you give an example of how they go above and beyond in educating patients?)
-I think that Drs. Chimes and Hyman both do a really good job of educating patients on what is going on with their bodies. When looking at the ultrasound, they understand that most people will not understand what they are looking at, so they go through the effort of explaining what exactly is being displayed on the screen. They will often use anatomical models to describe which structures are involved and how they are contributing to the problem. I think this helps patients understand the goals of their treatment options, and empowers them to follow through appropriately.  It takes extra time and effort to educate, but the patients clearly appreciate it. 
You are planning a career as a physical therapist.  What have your learned while working at Lake Washington Sports & Spine about how Dr. Chimes and Dr. Hyman integrate their care with the physical therapists they work with?
-Physical therapy is a treatment option that has proven very effective in recovering from a variety of musculoskeletal injuries. Having the correct diagnosis and following the appropriate protocols are key components that contributes to the efficiency of physical therapy. Drs. Chimes and Hyman are both incredibly talented diagnosticians, and are well versed in the strengths of various physical therapists, allowing them to best match a physical therapist's skillset with specific patients. Another element that Drs. Chimes and Hyman bring to the table is that they perform a number of different injections that can supplement physical therapy when the patient's progress has plateaued.

Complex injuries


I broke the car cable to my IPhone 5 the other day.  As I suspect is the case for many people, even though I intend to unplag the cable by grabbing the little black nubbin (.... I'm assuming "nubbin" is the technical term), in reality I probably yank on the cable more often than not.

Not surprisingly, it is starting to break, as you can see above.  To me, what is most interesting is wear it broke- at the interface between the nubbin and the casing.  It didn't have to break there- it could have broke in the cable itself, or the end of the nubbin that attaches to the casing, or the nubbin itself, or where the nubbin interfaces with the lightning plug.  It happened to break where it did, because along the chain that was the weak point.

It struck me that this was a perfect analogy for many of the musculoskeletal injuries Garrett and I see in clinic. Someone engages in some physical activity that causes an abnormal force across their body, and the body gets injured.  The place of injury can vary, though, depending on the specifics of their injury.

For example, the patellar ligament connects the patella (knee cap) to the tibial tuberosity (the little bump on the front of the shin bone), and this ligament complex can be injured from abnormal loading.  In this context, I am using complex to mean a linkage of things to one another, rather than as a synonym for complicated.  In 11 year old boys, the patellar ligament complex usually fails where the ligament attaches to the patella, something called Sinding-Larsen-Johansson syndrome.  In 14 year old boys, it usually fails where the ligament attaches to the tibia, called Osgood Schlatter syndrome, and in adolescents and young adults, it usually fails in the proximal 1/3 of the patella, called Jumper's Knee or patellar tendinopathy.

One of my job as a physician is to be familiar common loading patterns, and also recognize how things like age, gender, and common activities affect their different complexes, because they often have different injuries.

In the meantime .... fortunately my car cable still works.
